Due to Covid spacing requirements, we ask that only ONE parent/guardian come to registration, no need to bring your student at this time. Thank you!!
Please bring the following items with you to be copied:
*Immunization record
*Documentation of well-child physical
(Within the last 2 years)
*Birth Certificate
*Social Security Card
**If you do not have one or more of these items, please come register your child anyway so we can get him/her scheduled into a classroom. You may upload any missing items or bring them to the office during the summer.**
During your appointment, we will:
¨ Make copies of your documents (birth certificate, SS Card, etc.)
¨ Have you fill out forms, paper and online
¨ Answer any questions you may have about LDES Kindergarten
