Open to MHS students, Staff and staff's
spouses. Will be in the Bearcat Room.
This is 100% FREE! All students must
have a signed FERPA and ADH flu shot form
(which will be given out by enrichment teachers)
to participate in flu shot clinic. Forms have been
emailed to staff & staff spouses. Please get the
forms back to the nurse as soon as possible.
I accept faxed, e-mailed, and hand delivered
forms! The Polk Co Health Dept nurses will be
here in the morning only.
